Mental Health Services
Peer Support Services. Peer-to-peer support can help individuals overcome barriers and obstacles that, at times, feel insurmountable. Conveying empathy, using good listening skills, meeting individuals at the level they are at and treating individuals with respect, warmth, and dignity are aspects of quality peer support service delivery.
Targeted Casemanagement. This service provides a mental health casemanager that completes an assessment and develops a plan to help the child and family access services, obtain benefits and develop natural supports.
Psychiatric Outpatient. These are clinic based services provided by a therapist, psychologist or psychiatrist. Services can be delivered individually, or in a group.
Psychiatric Rehabilitation. The model currently available in this area is the Clubhouse. This is a daily program with a focus on activities that promote learning skills that could eventually support community employment. Members attend and work at the Clubhouse as they choose.
Mobile Outpatient. These are outpatient services that can be delivered to you in your home or in the community if certain conditions are met.
Building The A.B.C.'s. Building the ABC's (attachment, bonding and connections) uses the Promoting First Relationships approach, a positive, strengths-based reflective model to promote mutually enjoyable relationships between caregivers and their young children through mobile mental health treatment. Referrals for this program are coordinated through assessments by the York/Adams Early Intervention Program.
Partial Hospitalization. These are intensive clinic services that provide group and individual counseling as well as med-management. Individuals would receive several hours of therapy in any given day. The length of treatment could be several days, to several weeks.
Community Outreach & Recovery Essentials (C.O.R.E.) CORE uses an integrated approach which merges clinical and therapeutic staff - including a psychiatrist, nurse, mental health and substance abuse therapists - in one mobile service delivery team. While the team operates in a mobile capacity durng normal business hours, CORE also provides 24/7/365 telephone support for residential program and/or other behavioral health support staff involved with members.
Assertive Community Treatment (ACT). Assertive Community Treatment (ACT) is a voluntary, primary, direct service which provides a comprehensive and intensive approach to community-based mental health and addictions treatment, rehabilitation, and support services to persons with a serious and persistent mental illness and/or addiction problems. The ACT team includes - a psychiatrist, nurse, therapist, substance abuse specialist, vocational specialist and peer specialist. Assertive Community Treatment services are targeted for those persons who have not achieved and maintained health and stability in the community and for whom without these services would continue to experience hospitalization, incarceration, psychiatric emergencies and/or homelessness.
Extended Acute Care Program. A secure, structured residential setting that is staffed 24 hours a day, seven days a week. Intensive supports are built into the program utilizing a multi-disciplinary staff to provide individualized assessment, treatment and rehabilitation services. Medication management, meals and other personal living supports are also provided. The goal of this program is to help individuals to use systemic and natural supports to live in other community-based residential programs or independent living.
Inpatient Psychiatric (Hospitalization). This is overnight care in a hospital. This service is used if your problem cannot be treated outside the hospital to provide stabilization.
Drug & Alcohol Services
Drug and Alcohol Outpatient. These are clinic based services that are provided by a drug and alcohol therapist or doctor.
Intensive Outpatient (IOP). Delivered within a clinic setting, IOP is more intensive therapy that is provided several times a week.
Methadone Maintenance. This service uses a specific medication therapy to treat the member when addicted to certain kind of drugs. You may be eligible for certain visits associated with the administration of this medication. The behavioral health program does not pay for the medication, however your physical health benefit may fund this.
Suboxone (Buprenorohine). This is a medication used for opiate dependence. You may be eligible for certain visits associated with the administration of this medication. The behavioral health program does not pay for the medication, however your physical health benefit may fund this.
RASE Buprenorphine Coordinator. The Buprenorphine Coordinator Program developed by the RASE Project assists persons receiving buprenorphine to access recovery through a multi-faceted approach, including referrals to appropriate outpatient treatment providers and other essential services. The Program supports and monitors these individuals in their early recovery process, and continues to provide care to them to ensure that their issues are identified and addressed. The Program works directly with the individuals receiving the buprenorphine, doctors' offices, treatment service providers, and other ancillary service providers to assist persons new to recovery successfully navigate the system.
RASE Recovery Support Services Program. (Not an inplan service. Funded through reinvestment dollars.) The York/Adams Recovery Support Services Program (RSSP) has been designed to assist individuals who are in need of recovery services to assist them to overcome the obstacles that keep them from succeeding in the recovery process. All services offered through the RSSP will be directed at improving and increasing participants' recovery capital, level of life functioning, and the ability to sustain recovery.
Halfway House Services. Halfway House Services are provided in a residential setting which may be a step down from a higher level of drug and alcohol treatment. Members get jobs, and participate in community based treatment.
Non-Hospital Detox. This is a residential program that provides a safe place for withdrawal from drugs.
Non-Hospital Rehab. This is a residential program that provides for longer term treatment in a community based facility.
Hospital Based Detox and Rehab. This is overnight care in a hospital for drug and alcohol addiction. This service is used if your problem cannot be treated outside the hospital.
