Vision Statement

The Intellectual Disabilities program offers case management or supports coordination to individuals with a qualifying diagnosis of an intellectual disability, developmental disability (ages 3 to 8) or autism. The supports coordinator can link to services and supports, including family support services, community participation supports (formally called day programming), supported employment, residential habilitation, in-home and companion as well as respite. Referrals for intellectual disabilites services can be made by calling our office.
