Filing Instructions - This is the civil complaint form for use in filing small claims. Tenants also should use this form when filing complaints about landlords. You need to have one copy of the civil complaint with original signature for the magisterial district judge. You will be charged filing costs and service costs when the complaint is filed at the District Court. Please refer to the Fee/Cost Schedule for approximate costs. Filing Instructions - This form is to be used by private citizens when requesting criminal charges be filed against a defendant. Please note that Rule 506 of the Pennsylvania Rules of Criminal Procedure requires that when a private citizen completes this form, it must be submitted to the District Attorney's Office for approval before it can be filed in a Pennsylvania Magisterial District Court. In York County, Summary Bad Check complaints Section 4105 Sub-section A1 of the PA Crimes Code (1st or 2nd offense and the check value is <$200.00 per check) may be filed without prior DA approval.

A Creditor who was awarded a Civil Judgment by the Magisterial District Judge shall file this form with the District Court upon satisfaction of the Judgment. If the Creditor fails to file this form in a timely manner after the Judgment has been satisfied, the person to whom the Judgment was against, may file this request.
